THE GREATER ZION MISSIONARY BAPTIST CHURCH CONSENT AND RELEASE FORM

The undersigned parent or guardian hereby consents to allow named child/ person above, to participate in all activities of the VACATION BIBLE SCHOOL, JULY 15-19, 2019, SCHOOL OF CHRISTIAN EDUCATION MINISTRY of the Greater Zion Missionary Baptist Church. During the classes and activities of this ministry, the children will be expected to take part in the academic, spiritual and physical learning, indoor and outdoor recreational sports and games, snacks and other activities as part of the ministry. I certify that my child is able to participate in these events. I hereby give permission for photographs and other media materials to be used for promotional display purposes and local media and news coverage. If my child has medical conditions which may be relevant to a physician in the event of an emergency, I have listed the medical conditions below. In the event an emergency occurs, I may be reached at the telephone number listed below. If I cannot be reached, I hereby authorize Directors, Instructors and the Pastoral Staff to make emergency medical decisions, for my child. If there are any activities I do not want my child to be involved in, I have listed them below. I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL OF THE RISK WHICH MAY BE ENCOUNTERED ON SAID ACTIVITY, INCLUDING ACTIVITES PRELIMINARY AND SUBSEQUENT THERETO: I do hereby agree to hold The Greater Zion Missionary Baptist Church and it’s agents, volunteers and employees, harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which I now have or which may arise in the future in connection with the activity or participation in any other associated activities. I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by law of the State of Pennsylvania and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto and the terms of this release are contractual and not a mere recital. I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE OF MY OWN FREE WILL. This is a legally binding agreement which I have read and understand.

The Greater Zion Missionary Baptist Church 2019 Vacation Bible School (VBS) Discipline Behavior Guidelines

VBS Director and Staff are required to treat each student with courtesy, respect, and with an understanding of individual needs. Corporal punishment is not the policy at VBS. Non-physical behavior modification will be used unless physical restraint is needed to protect VBS STUDENTS FROM HARM. In the event of a violation of the VBS Behavior Guidelines VBS Teachers and Staff will use the most appropriate approach to redirect and correct inappropriate behavior situations. VBS Student Behavior Guidelines WE ARE TO BE: 1. Respectful to staff by addressing them with titles (Ms. Mr. etc.) 2. Respond to staff with respect (Yes or No Sir & Yes or No Ma’am)  We do not use: YEAH, HUH or WHAT 3. Respectful to other students 4. Respectful of the church and all within 5. Without any electronics (disc-mans, Walkman, hand held games, cell phones and IPod) 6. Free of Drugs and or Alcohol The following behaviors will not be tolerated under any circumstances: NO pushing, shoving or fighting NO disrespectfulness towards staff NO throwing objects NO name calling NO harassment, intimidation or bullying towards others. NO leaving VBS classrooms or the church without permission NO stealing NO foul or profane language NO vandalism NO smoking or use of drugs and or alcohol NO weapons of any form The breaking of any of these rules can lead to removal from the program. I have read and agree to adhere to the above Discipline Behavior Guidelines

The Greater Zion Missionary Baptist Church 2019 Vacation Bible School (VBS) Youth Expectations

VBS is a special event that creates memories that help lock Bible truths in a child's heart and mind for a lifetime. Birthday parties and other special events have themes that use creativity to evoke feelings of excitement. Games, food, decorations and fun all link together in a total package. VBS does the same thing. When we connect with kids through this ministry, the biblical truths that permeate all aspects of VBS also connect with their hearts and minds. Please review this expectations form with your child/ youth WE ARE TO: 1. Attend Vacation Bible School with a smile 2. Attend with a positive attitude and willingness to participate. 3. Please show respect for others and their property. 4. When in doubt, ask questions of your teacher or VBS support staff. 5. Please be aware youth are responsible for their personal belongings. It is best not to bring expensive or important property to VBS 6. Please follow all safety rules 7. HAVE FUN! Incidents and behavior concerns will be addressed with parents on a case-by-case basis. I have read and agree to adhere to the above Youth Expectations: Discipline Behavior Guidelines.
