BACK TO SCHOOL BASH - BOOK BAG VOUCHER REQUEST FORM

The Book Bag Voucher will be emailed to you within 24 - 48 Hours of the completed form. We do have a limited number of book bags and want to ensure we are able to bless as many students as possible with back to school giveaways. We ask that you present physical or Email copy of voucher and number at the time of pick up. We also ask that no one sign up for a voucher that will not be in attendance on August 17, 2019. Book Bags will only be given out to students who are present to receive them. If at 3:30 pm book bags are not picked up we will then give them to students present that may not have a voucher on a first-come, first-serve system. Before submitting this Voucher Request Form please acknowledge that you have read and understood the aforementioned details by checking yes in the field to the right.
